As the second-largest Ontario Health Teams (OHT) in the province, the Central West OHT serves nearly 900,000 residents—and primary care is at the heart of our vision.
With over 275 health and community partners already collaborating—including acute care, home and community care, specialty care, long-term care, mental health and addictions services, and community support services—we are building a system where primary care providers are empowered and supported to lead integrated care for their patients.
