Toronto Public Health’s Healthy Babies Healthy Children (HBHC) home-visiting program has relaunched its Online Referral Form for Regulated Healthcare and Social Service Providers. TPH is hoping the updated referral process will help increase prenatal referrals and improve access to services for those who may benefit from early support.
Services are free and no OHIP is required.
HBHC provides support to individuals and families in the following areas:
- Prenatal (General, Nurse Family Partnership [NFP] and Homeless at Risk Prenatal)
- Postpartum
- Parenting
