The Central West Ontario Health Team (CW OHT) is pleased to announce that Dr. Christine Cho has been selected as the new Primary Care Lead for Older Adults, playing a critical role of fostering collaboration amongst community providers for care for older adults, particularly those experiencing frailty.
About Dr. Christine Cho
Dr. Christine Cho is a family physician who has worked in comprehensive family medicine for ten years.
She completed medical school at the University of Calgary, and her family medicine residency at McMaster University’s Brampton location.
In addition to care in-office, Dr. Cho also provides home palliative care, and is especially passionate about providing quality care to her patients in the community.
She is also focused on teaching the next generation of family physicians, including by supervising medical students. Dr. Cho is currently the Brampton site Palliative Mentor for McMaster Family Medicine.
About the Role
The CW OHT’s new Primary Care Lead for Older Adults will serve a key role in strengthening collaboration among community providers to support the care of older adults, especially those living with frailty.
In this position, Dr. Cho will act as Co-Chair for the CW OHT’s Older Adults Planning Table and lead the development and implementation of a multi-year plan for Older Adults.
The role will include being a champion for integrated initiatives in the plan and other older adult programs already implemented, such as the CARE Program, which provides wrap around interdisciplinary care to home bound older adults experiencing frailty.
The Primary Care Lead for Older Adults will offer clinical insights and work alongside the OHT Secretariat and key OHT partners as needed. The individual will also be a key liaison with the CW Primary Care Network and its Council.
