The Central West Ontario Health Team (CW OHT) is pleased to announce that Dr. Marian Chen been selected as the new Primary Care Lead for Palliative Care, playing a critical role in representing primary care providers within the Palliative Care Planning Table, ensuring alignment with the CW Primary Care Network (PCN) priorities.
About Dr. Marian Chen
Dr. Chen is a palliative care physician at William Osler Health System.
She completed Palliative Medicine Enhanced Skills training at the University of British Columbia, following Family Medicine residency at the University of Toronto.
Dr. Chen provides both hospital-based and community palliative care across the Central West region and is deeply committed to advancing integrated, patient-centered palliative care.
Additionally, she has a strong interest in medical education, health system integration, and fostering collaboration across care teams.
About the Role
The CW OHT’s new Primary Care Lead for Palliative Care will support the implementation of the OHT’s multi-year Palliative Care Plan by working with the OHT Secretariat and key partners to plan and design initiatives.
The individual will act as a representative for primary care and ensure recommendations are aligned to the goals and objectives of the CW PCN.
This includes championing community interprofessional primary care team (IPCT) coordination, and planning for the attachment of the individuals on the CW Health Care Connect wait list.
This role will offer key insights and work alongside the OHT Secretariat and key partners, acting as a liaison with the CW PCN, Primary Care Council, and, where necessary the Collaboration Council.
