We are pleased to share that the Ministry of Health has launched a new, third round of Interprofessional Primary Care Team (IPCT) Call for Proposals for 2027-28.
This funding will create and expand approximately 78 primary care teams to connect 600,000 more people to primary care. This is part of the $3.4 billion that the province is investing to support Ontario’s Primary Care Action Plan. This will connect everyone in the province to primary care by 2029.
You can find more information here: Call for proposals for interprofessional Primary Care Teams
This Call for Proposals aligns with the broader Central West OHT Primary Care Attachment Plan to bring more primary care closer to home and ensure every resident is attached by 2029.
Through the previous two rounds of funding, the CW OHT has been successful in creating seven IPCTs in our region, which have attached over 30,000 patients to primary care so far. With an estimated 140,000 people previously unattached to primary care in our region, this represents a significant step towards improving access to care for our communities.
We are excited to continue to collaborate with you to submit a coordinate approach for our region by November 20, 2026. If you are interested in participating, please review the below materials and email us at [email protected].
