The Central West Primary Care Network (CW PCN) is a collaborative network of family physicians and healthcare professionals working together to deliver accessible, patient-centered primary care across Brampton, North Etobicoke, West Woodbridge, Malton, and Bramalea.
Rooted in a shared commitment to excellence, equity, and innovation, CW PCN supports primary care providers through team-based care models, clinical supports, and system navigation. Our work is grounded in local needs and guided by a vision for healthier communities where every person can access timely, comprehensive care close to home.
What We Do
Support Providers: We provide tools, programs, events, resources and allied health team members that help primary care providers better serve their patients.
Enhance Access: We work to reduce barriers to care, including through after-hours clinics, mental health supports, chronic disease management, and virtual care.
Coordinate Care: We collaborate with hospitals, home care, public health, and community services to ensure patients receive seamless and integrated care.
Drive Improvement: We use data and quality improvement strategies to enhance outcomes and experiences across our network.
